About Social Prescribing

Social prescribing is a holistic approach used in the UK healthcare system to support individuals with non-medical needs that affect their health and wellbeing.

Instead of offering medication or clinical treatment alone, healthcare professionals refer people to social prescribing, which helps them access community-based services and activities tailored to their personal goals and challenges. These services might include:

  • Arts and crafts groups
  • Gardening or walking clubs
  • Debt advice or housing support
  • Volunteering opportunities
  • Mental health peer support

The aim is to improve overall wellbeing by addressing social, emotional, and practical needs, especially for people who are:

  • Living with long-term conditions
  • Experiencing loneliness or isolation
  • Facing low-level mental health issues
  • Dealing with complex social challenges

Social prescribing is a key part of the NHS’s Universal Personalised Care strategy and is designed to reduce pressure on clinical services while improving outcomes through community connection and empowerment.

About Linking Leeds

Linking Leeds is an integrated, free city-wide Social Prescribing service for people in Leeds.

In Leeds, as well as their being a great team of Social Prescribing Link Workers in most Primary Care Networks, there is also a city-wide service for social prescribing, Linking Leeds.

Linking Leeds provides social prescribing for all people aged 16 years and above, no matter where they live in the city. We also work with the Primary Care Networks in responding to the specific health profiles of communities.

Aims of the service

Our aim is to connect people to services and activities in their community, to benefit overall health and wellbeing.

We believe that everyone can live a fulfilled life, however, sometimes people need help to achieve this.

Feeling linked to your community has lots of health and wellbeing benefits, it supports resilience, independence and good physical and mental health.

So, if practical or emotional problems are stopping you feeling your best, Linking Leeds can help you get them sorted.

Our Wellbeing Coordinators work within GP practices providing one-to-one support face to face. Community outreach happens in various locations. You can also be supported over the phone or online. The choice is yours!
