Getting support from Linking Leeds

Anyone can let us know of someone who needs support including the person themselves. We take introductions from GPs and other staff within the surgery, from other services and health professionals as well as friends and family of the person with their consent.

Often when you are working with someone, it is hard to address all of their needs. This is where we come in. By getting a full picture of their situation, we will be help  them to access all of the support they need by doing a full assessment and setting up an action plan with them. This can help you to focus on your area of expertise and lets us help support with their other needs.

We are a very inclusive service. To get support from us you must be 16 years or older and registered with a GP in Leeds.

Information for GPs/surgery staff:

You can use our remote booking system on SystmOne to introduce clients to us. Alternatively you can fill in the introduction form. Remote booking can also be accessed on EMIS via extended hours hub. If you have identified that one of your patients would benefit from our help, please just let us know why this is.

We will also do a full assessment with the person to understand all of their support needs. If you do not have the information about remote booking, please ‘contact us’ and we will send it to you. You can also ask for support with this from your Wellbeing Coordinator.

Information for other services:

Information for people who need help:

You don’t have to know exactly what you need support with. We will help you to understand that. In your first session with us we’ll ask you some simple questions. There is nothing scary about this. Our Wellbeing Coordinators are trained to make this process as easy as possible. They are friendly and will listen without judgement, their only aim being to help you. No matter what your situation, we’ll do our best to support you to improve on the things that matter to you.

We will take it at your pace and we will not force you to act on anything you don’t want to. Working with us is about helping you to solve the issues that matter most to you. We will guide you through the process every step of the way.

Please fill in this form to refer yourself. It is quick and simple to fill in. Your Wellbeing Coordinator will give you a call soon after this to introduce themselves and explain more about the service. They will also set up an appointment with you to do your assessment.
