On SystmOne, you can use our remote booking system to directly book an appointment for a patient to speak with a Wellbeing Coordinator. You can use submit an electronic referral using the blue dot template. Remote booking can also be accessed on EMIS via extended hours hub. Alternatively you can fill in the referral form on this website.

If you have identified that one of your patients would benefit from our help, please just let us know why this is, it is useful to have as much information as possible in order to prevent a patient needing to repeat themselves. We will also do a full assessment with the person to understand all of their support needs.

If you do not have the information about remote booking, please contact us and we will send it to you. You can also ask for support with this from your Wellbeing Coordinator. Find out who they are here.

Information for other services:

As a professional, often when you are working with someone, it is hard to address all of their needs. This is where we come in. By getting a full picture of their situation, by doing a full assessment and setting up an action plan, we will be help them to access all of the support they need. This can help you to focus on your area of expertise, with other needs being met. If you are working with someone who you think would benefit from our support, please encourage them to self-refer.

We do not accept referrals from professionals outside of Primary Care. If you are working with someone who you think would benefit from social prescribing, please encourage them to self-refer.

Information for people who needs help:

You don’t have to know exactly what you need support with. We will help you to understand that. From your first session with us we’ll ask you simple questions that will guide us in figuring things out. We understand that it can feel daunting to share how you are feeling with a new person. Our Wellbeing Coordinators are trained to make this process as easy as possible. They are friendly and will listen without judgement, their only aim being to help you. No matter what your situation, we’ll do our best to support you to improve on the things that matter to you.

We will take it at your pace and we will not force you to act on anything you don’t want to. Working with us is about helping you to solve the issues that matter most to you. We will guide you through the process every step of the way.
