Meet Our Team

The Service Manager is responsible for the running, improvement and performance of Linking Leeds. The Service Manager works with key partners and stakeholders across the city to support working relationships and lead on system improvements.

Primary Care Network Managers, Practice Managers and other key partners across Leeds might contact the Service Manager to discuss issues, to give feedback or if they want to work more closely with the service. The Service Manager is responsible for managing the Locality Managers and the Hub admin team.

Linking Leeds is a city wide service, delivered locally. We have clustered the Primary Care Networks into three areas based on demographics. Each Locality Manager has responsibility for an area cluster of Primary Care Networks. They work directly with Primary Care Network leads, Practice Managers and Local Care Partnerships. They manage a team of Wellbeing Coordinators in their area to ensure that the service is delivered effectively.

The Linking Leeds Hub is within the Reginald Centre in Chapeltown. There we have a fantastic team of administrators who provide vital support for the day-to-day running of our service. This team answer the phone, manage queries from professionals and people wanting to access the service, process referrals, share information, respond to website queries and generally keep the Hub running effectively.
